At Heritage Eye, Skin & Laser Center, we recognize how big of a challenge it may be to pay up front for various treatments and procedures, especially those not covered by insurance. That is why we are pleased to announce that financing is now available for many of the elective procedures and services offered in our office. Whether you’re considering LASIK, BOTOX® Cosmetic, or skin treatments, we’ve got you covered. We offer multiple financing options through Care Credit. The application process is simple and quick (usually less than 10 minutes) and most patients receive a response almost immediately.

For your convenience, we have made a number of forms available for patients to download. If you are new to our practice, we encourage you to download and print out the new patient forms. Filling these out in advance of your first visit will minimize the time you spend in the reception area filling them out when you get here. If your doctor has given you a form with specific information or instructions different from the information on these online forms, rely on the information given to you by your doctor, not on the online forms.
